1. Who we are
Chiswick Knee Clinic is a trading name of Sports Healing Ltd, which is registered in England and Wales (company number 05540087) and is the controller of your personal information. Our registered office is Woodlands Grange, Woodlands Lane, Bradley Stoke, Bristol BS32 4JY, and we see patients at Chiswick, the Wellington Hospital and 75 Harley Street, London. We are registered with the Information Commissioner's Office under registration ZA005873.

2. The information we collect
When you use the appointment request form, we collect your name, date of birth, phone number, email address, and the reason for your appointment. When you contact us by phone or email, we keep a record of that correspondence. When you visit the website, we may also collect limited technical information through cookies and analytics.
3. Health information
The reason you give for your appointment may tell us about your health. Health information is a special category of personal data under UK data protection law, and we treat it with extra care. We only use it to respond to your enquiry and to arrange and provide your care.
4. Why we use your information, and our lawful basis
We use your information to reply to your enquiry, arrange your appointment, provide clinical care, and keep proper records. Our lawful bases under the UK GDPR are:
- your consent, which you give when you submit the form (Article 6(1)(a));
- taking steps at your request before entering into a contract for care, and our legitimate interest in responding to enquiries (Article 6(1)(b) and (f));
- for health information, your explicit consent and the provision of health care (Article 9(2)(a) and (h)).
You can withdraw your consent at any time by contacting us. This does not affect anything we did before you withdrew it.
5. Who we share it with
We share your information only where we need to in order to help you. This may include the treating consultant (Mr Chinmay Gupte) and clinical team, the hospitals and diagnostic centres where you are seen (including HCA UK sites), and trusted service providers who process information on our behalf, such as our website form provider and our appointment booking system. We require these providers to protect your information and to use it only for the purposes we set. We do not sell your information or share it for marketing.
6. How long we keep it
We keep enquiry information for [retention period, for example 12 months] unless you become a patient, in which case your clinical records are kept in line with professional and legal requirements for medical records [state the retention period you apply]. After that, we securely delete or anonymise it.
7. How we protect your information
Information you send through the website is transmitted over an encrypted connection. We limit access to your information to the people who need it, and we keep appropriate technical and organisational measures in place to guard against loss, misuse or unauthorised access.
8. Your rights
Under UK data protection law you have the right to ask us to give you a copy of your information, correct it, delete it, restrict how we use it, or object to our use of it, and in some cases to receive it in a portable format.
